Inspector Notes

This inspection was conducted by licensing staff using a hybrid protocol, including reviewing some records remotely and an on-site inspection.

A renewal inspection was initiated on February 9, 2022 and was completed on February 18, 2022. On February 9, 2022, there were 59 children present, ranging in ages from 5 months to 7 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies, and nutrition. A total of 4 children's records, 4 complete staff records, and 6 partial staff records were reviewed.

Information gathered during the inspection determined non-compliances with applicable standards or law and the violations were documented on the violation notice issued to the program.

Based on staff record review and discussion with the director, one staff member had not obtained a required background check.

Evidence:
1. The record for Staff #5 did not contain a completed Criminal History Record Check. Date of hire was 01/03/2022.
2. The director confirmed that the fingerprint criminal history check had not been completed for Staff #5.
Plan of Correction: An appointment was made to get the fingerprint check completed and Staff #5 did not return to work until the results were received. The completed check was received on 02/23/2022.

Based on staff record review and discussion with the director, an out-of-state background check had not been completed for one staff member.

Evidence:
1. The record for Staff #9 did not contain an out-of-state Sex Offender Registry Search. Date of hire was 03/19/2021.
2. The director confirmed that the sex offender check for Staff #9 had not been completed at the time of hire.
