Inspector Notes

A monitoring inspection was initiated on 12/14/2023 and concluded on 12/14/2023 from 10:50 AM to 12:20 PM. There were 60 children present, ranging in ages from eight months to eight years, with 16 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of six children?s records and the six staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to ensure staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:

1. The record for staff 1 documented employment date as 5/15/23. There was no documentation of staff 1 completing the Virginia Department of Education-sponsored orientation course.
2. The record for staff 2 documented employment date as 5/22/23. There was no documentation of staff 2 completing the Virginia Department of Education-sponsored orientation course.
3. The record for staff 3 documented employment date as 8/30/23. There was no documentation of staff 3 completing the Virginia Department of Education-sponsored orientation course.

Standard 8VAC20-780-280-B
Based on observation, the center failed to keep hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children.

Evidence:

In the restroom used by children in the downstairs restroom, two spray bottles of cleaner were hanging by yarn attached to carabiners behind the toilet. The children in the classroom were ages three to eight.
Plan of Correction: The hazardous substances were moved to a locked location.
