Inspector Notes
An unannounced monitoring inspection was initiated on January 16, 2025 and completed on January 16, 2025 in response to a self-report received by the licensing office on December 20, 2024 relating to toileting procedures. The inspector reviewed one staff record. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 30, 2025. A POC submitted after this date will not appear on the public website.
Violations
2On December 19, 2024, while diapering child #1 (age five months) staff #1, who knew child #1 could roll over, stepped away from the counter the child was lying on, while not looking at, nor touching the child, to talk to a staff member. Child #1 fell approximately 3 feet to the laminate floor hitting their head on a cushioned mat and body on the bare floor, resulting in the child crying and having a red mark on the child's back.

On December 19, 2024, a five month child fell approximately three feet from the counter top while being diapered to the laminate floor. The child's head hit a cushioned mat and body hit the bare floor, resulting in the child crying and having a red mark on their back.
