During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated on January 16, 2025 and completed on January 16, 2025 in response to a self-report received by the licensing office on December 20, 2024 relating to toileting procedures. The inspector reviewed one staff record. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 30, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
REPEAT VIOLATION When staff are supervising children, they shall always ensure their care, protection, and guidance.
On December 19, 2024, while diapering child #1 (age five months) staff #1, who knew child #1 could roll over, stepped away from the counter the child was lying on, while not looking at, nor touching the child, to talk to a staff member. Child #1 fell approximately 3 feet to the laminate floor hitting their head on a cushioned mat and body on the bare floor, resulting in the child crying and having a red mark on the child's back.
Plan of Correction: We immediately debriefed after the incident. The teachers had shifted their body to the side and the child fell between the adults body. We updated all our guidance to be sure that staff know to keep their hand on the child on the changing table
anytime they are not looking at the child. We added a new policy for new teachers to be observed after orientation and given feedback.
Standard 8VAC20-780-500-B
Staff shall ensure the immediate safety of child during diapering.
On December 19, 2024, a five month child fell approximately three feet from the counter top while being diapered to the laminate floor. The child's head hit a cushioned mat and body hit the bare floor, resulting in the child crying and having a red mark on their back.
Plan of Correction: We immediately debriefed after the incident. The teachers had shifted their body to the side and the child fell between the adults body. We updated all our guidance to be sure that staff know to keep their hand on the child on the changing table
