During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring and focus inspection was initiated on September 18, 2025 and completed on September 18, 2025 as part of probation. The department issued a Special Order on July 18, 2025 for Probation and Required Licensee to Contact Parents, Guardians or Other Responsible Persons Regarding Health and Safety Violations on March 26, 2025 as a result of high-risk violations. The on-site inspection began at 10:35 a.m. and ended at 11:15 a.m. The inspector reviewed compliance in the areas listed above. There were 70 children present and 11 staff. The inspector reviewed seven children?s records and seven staff records electronically on September 19, 2025. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
