Inspector Notes
An unannounced, on-site monitoring inspection was completed on 03/03/2025. The on-site inspection began at 10:56 a.m. and ended at 3:30 p.m. The inspector reviewed compliance in the areas listed above. There were 122 children present and 20 staff. The inspector reviewed 10 children?s records and 9 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/28/2025. A POC submitted after this date will not appear on the public website.

7The center is required to obtain background checks from any state in which the individual
has resided in the preceding five years. S1, employed for more than 4 months, did not have
documentation of the sexual offender registry check and central registry check. S4, employed for more than 5 months, did not have documentation of the central registry check.
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of
employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. TB documentation for S1 was dated 43 days prior to date of employment. TB documentation for S4 was dated 33 days prior to date of employment.

The center is required to maintain documentation of emergency evacuations that
document all of the required information. The 2024 shelter-in-place drills and lockdown drill did not document: the method used for notification for the drill; the number of staff participating; any special conditions simulated; and problems encountered, if any.
