An unannounced, on-site monitoring inspection was completed on 06/04/2025. The on-site inspection began at 8:58 a.m. and ended at 12:05 p.m. The inspector reviewed compliance in the areas listed above. There were 53 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/12/2025. A POC submitted after this date will not appear on the public website.

For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service. S1 did not have central registry search results within the required time frame.

A tuberculosis (TB) screening is required every two years from the last screening. S1 updated screening was due 1 month ago. The updated screenings have not yet been completed.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. S1 and S2, who are working with children, have not yet completed this course which is overdue by 2 years.

Staff shall complete orientation training required by 8VAC20-780-240-C and 8VAC20-780-420-A prior to working alone with children and no later than seven days of the date of assuming job responsibilities. The record for S1 did not contain an orientation training form.

Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care. The records for S1 and S2 did not document the 16 hours of required training for 2024.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The wooden ship in the children?s playground is rotting and splintering. Two fence post concrete footings are exposed on the playground.

The center shall maintain written records of children?s injuries that contains all of the
required information. Injury reports dated 04/18/2025 and 05/28/2025 did not document how parents were notified of the injury and the time of notification. Injury reports dated 05/06/2025 and 05/16/2025 did not document the time of parental notification. The injury report dated 05/06/2025 did not document a second staff signature or parent signature.
