Inspector Notes

An unannounced monitoring inspection was initiated and completed on 02/20/2026 in response to a self-report received by the licensing office on 01/23/2026 relating to supervision. The inspector reviewed 1 staff record and 1 child record onsite. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/30/2026. A POC submitted after this date will not appear on the public website.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.
On 01/23/2026, 3-year-old, C1 was not within actual sight and sound supervision of staff for over 4 minutes after being left alone on the playground.
Plan of Correction: Teacher is no longer employed with us. We followed our policy and reviewed policy with staff members.
