Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 03/18/2021 and concluded on 03/22/2021. The director was contacted by telephone to initiate the inspection. There were 36 children present and ten staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Violations
2Evidence: 1) Staff #2 was hired on 03/09/2021. The fingerprint-based criminal history check determination letter is dated 03/10/2021.
2) Staff #3 was hired on 03/12/2021. The fingerprint-based criminal history check determination letter is dated 03/15/2021.
3) During interview, a member of management acknowledged the fingerprint-based criminal history check determination letters for Staff #2 and Staff #3 were not received prior to hire.
Evidence: 1) The sworn statement in the record for Staff #2, hired on 03/09/2021, indicated the staff member lived out of state within the last five years. 2) The record did not contain the results of a sex offender registry check. 3) During interview, a member of management reported the center did not conduct the required out-of-state check for Staff #2 prior to hire.
