During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/16/2024. The on-site inspection began at 10:00am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 44 children present and 10 staff. The inspector, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record for Child #2 contained documentation of immunizations obtained four days after the child began attending the center.
Plan of Correction: We will ensure going forward all children do not attend the center prior to submitting required information.
Standard 8VAC20-780-60-A
Centers must keep a separate record for each child enrolled that contains all of the required information. Child #1's record (enrolled eight months) was missing one of the two required names, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: We will ensure going forward all children do not attend the center prior to submitting required information.
