Inspector Notes
A mandated monitoring inspection was conducted on November 15, 2023 from 9:45 A.M.-1:00 P.M. There were 83 children present, ranging in ages from 2 years 2 months to five years of age, with twelve staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight child records and eight staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
5Evidence:
In the Pre-K A classroom there are several areas under the calendar and white board that has several areas where there is peeling paint. The peeling paint is accessible and at the children's level.
Evidence:
The area around the large play structure on the playground, and at the chute of the slide did not contain 6 inches of resilient surfacing; bare ground was visible in multiple areas around the playground structure and at the end of the slide chute.
Evidence:
Child #1 had an a bottle of Children's Benadryl on site. The medication authorization signed by the physician expired on 08/02/2023.The medication had not been administered.
Evidence:
1. Child #3 had a bottle of Children's Benadryl on site. The medication expired on 04/2023.
2. Administration verified that the non-prescription medication had expired.
Evidence:
1. In the 3/4's classroom there was a lunchbox that was not dated.
2. In the 3's classroom there were three snacks in the snack cart that were not dated.
3. Administration verified that food brought in from home was not dated.
