Inspector Notes

An unannounced inspection was initiated on 09/30/2025 and completed on 10/03/2025 in response to a complaint received by the licensing office on 09/25/2025 relating to allegations of staff and supervision, and special services. The inspector reviewed 0 children?s records and 1 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

When staff are supervising children, they shall always ensure their care, and protection.

One September 16, 2025, staff transported nineteen four-year-old children in a 14-passenger bus, not only exceeding the maximum number of passengers, but seating more than one person in seats having only one seatbelt.

The center shall ensure that during transportation of children: Virginia state statutes about safety belts are followed as required by ?? 46.2-1095 through 46.2-1100 of the Code of Virginia and stated maximum number of passengers in a given vehicle is not exceeded.

Staff 2 confirmed that 21 persons were transported on a 14-passenger bus on a field trip on September 16, 2025, nineteen of which were Pre-K students (4 years old).
