Inspector Notes

A monitoring inspection was conducted on 11/8/23 from 9:50 am to 11:20 am. There were 5 children (13 points) present with 3 caregivers supervising. The children were observed during circle time, learning the letter V, and playing outdoors.There were no children with medications on site. 2 children's records, 3 caregiver's records, emergency drills and attendance records were reviewed.

Repeat Violation
Based on records review, the provider did not obtain results of a fingerprint based criminal record for employees prior to the first day of employment.
Evidence:
1. Staff # 1 first day of employment was 8/21/23.
2. Staff # 1's fingerprint results were obtained on 8/30/23.
Plan of Correction: Fingerprints will be completed before the first day of employment for all new employees.
Standard 8VAC20-800-120-B
Based on records review, the provider did not obtain two written reference checks for an assistant prior to the first day of employment.
Evidence:
1. Caregiver # 1's first day of employment was 8/21/23.
2. The provider did not obtain 2 reference checks for Caregiver # 1 to this date.
Plan of Correction: References will be completed and kept on file.
Standard 8VAC20-800-240-A
Based on observation, furnishings inside the house were not maintained in a safe condition.
Evidence: The right cabinet door for the diaper changing table has a broken corner posing an injury hazard for the children.
Plan of Correction: The corner will be repaired or covered.

Repeat Violation
Based on observation, soiled disposable diapers were not being disposed off in a plastic-lined storage system.
Evidence: Two foot pedal diaper pails used for diaper disposal were not lined by plastic trash bags.
Plan of Correction: The diaper pails were lined with bags during the inspection.
