Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 10/18/24 from 9:15 am to 10:40 am. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers; the point total was 10. The inspector reviewed 2 children?s records and 2 caregiver's and 1 household member's records on-site. This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The family day home must obtain results of a central registry search (CRS) for adult household members.

The provider stated that Household Member # 1 started residing in the home approximately 3 months ago (August 2024). The CRS for Household Member # 1 has not been obtained to this date.
Plan of Correction: A follow up will be conducted for the CRS or a new one will be submitted. I will follow up with my licensing inspector.

REPEAT VIOLATION

Records for assistants shall include documentation of two reference checks.

Caregiver # 1's first day of employment was 9/30/24. Only one reference check was completed for Caregiver # 1.
Plan of Correction: A second reference check will be obtained.
