During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 10, 2025 and completed on September 10, 2025, as a part of the current licensure period. The on-site inspection began at 11:30am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 20. The inspector reviewed 5 child files and 2 caregiver files on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 17, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

Updated TB screenings for Caregiver 1 and 2 were completed 2 months after the previous screening expired.
Plan of Correction: We thought TB screening every 2 years was sufficient and had no idea about continuity. We'll make sure of no lapse in continuity from now on.
Standard 8VAC20-800-680-B
Caregivers shall ensure that children's hands are washed with liquid soap and warm running water, before and after a diaper change.

Caregiver 1 was observed changing a child's diaper and did not wash the child's hands before or afterward.
Plan of Correction: She had a dry diaper that wasn't soiled and didn't need to be changed. I was just checking her because of her wet shirt (due to teething).

The diapering surface shall be cleaned and sanitized after each use.

Caregiver 1 was observed changing a child's diaper and did not clean or sanitize the diapering surface afterwards.
