During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 31, 2026 and completed on March 31, 2026, as a part of the current licensure period. The on-site inspection began at 12:30pm and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 caregivers; the point total was 25. The inspector reviewed 1 child record and 2 caregiver/household member records. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

The physical exam record available for Child 1 was dated more than 90 days after the child's first day of attendance.
Plan of Correction: We will have parents give us the documents before they are due.
