Inspector Notes

A monitoring inspection was initiated on 8/18/2022 and concluded on 8/18/2022. There were a total of 8 children (31 points) observed with 2 caregivers present. The children were engaged in the following: toy play, games, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 8 child records and 3 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-590-B
Based on observation, a child who falls asleep in a play space other than their own crib, cot, mat, or bed were not moved promptly to their designated sleeping space.

Evidence: Two children were sleeping in infant rocking equipment and were not promptly moved to their designated sleeping space.
Plan of Correction: Provider moved children to their cribs and will promptly move children to their sleeping area if this happens again.
Standard 8VAC20-800-90-C
Based on file review, the family day home did not obtain documentation of additional immunizations for a child once every six months for children under the age of two years.

Evidence: There was no documentation that Child #1 was up to date on their vaccinations.
Plan of Correction: Provider will ask parent for updated vaccine record and keep on file.
