The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #2, #4 and #5, who had been in care for over 60 days.
Plan of Correction: The provider will obtain records of physical examination for the children.
Standard 8VAC20-800-60-B
Children's records are required to contain parental employment addresses and phone numbers. The record for Child #2, and #3 was missing the employment phone number for one parent. The children were present during the inspection and had been in care for over one year.
Children's records are required to contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The record for Child #1 was missing the second emergency contact. The record for Child #5 was missing both emergency contacts. The children were present during the inspection.

Documentation shall be maintained of monthly emergency evacuation drills. The provider did not have documentation of the monthly emergency evacuation drill for the month of December.

Documentation of additional immunizations is required once every six months for children under the age of two years. Child #2, eighteen months old, last documented immunization was over one year ago. Child #4, twenty-eight months old, last documented immunization was over fifteen months ago.
