Inspector Notes
An unannounced monitoring inspection was conducted from 10:45 am to 12:05pm. There were 12 children (45 points) with 4 caregivers present. The children were observed playing and engaging with the caregivers. Diapering and nap time preparation was observed. Infant bottles, first aid kit and the emergency drill log was reviewed. 5 children's records, 3 caregiver's records and 1 household member's record was reviewed. There were no medications on site.

Violations
4Evidence:
1. Household member # 1 turned 14 years old on 9/15/22.
2. The central registry results for Household member # 1 were dated 1/9/23.
3. The provider stated that the request was submitted and sent back due to incompletion. However, there was no documentation to show if the request was actually submitted on time.
Evidence: Caregiver # 1's first day of employment was 9/26/22. Letters of references for Caregiver # 1 were dated 10/2/22 and 10/4/22.
Evidence:
1. At the beginning of the inspection at 10:45 am, the provider was not at the family day home and there were 12 children with 3 assistants present.
2. The provider returned to the family day home at approximately 10:55 am.
3. None of the assistants present had a current certification in cardiopulmonary resuscitation (CPR).
Evidence:
