Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 7/23/25 from 10:00 am to 10:30 am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 3 caregivers; the point total was 19. The inspector reviewed 3 children?s records and 3 caregiver's records on-site. This inspection included document review, tour of the family day home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The provider shall obtain for each caregiver a current Report of Tuberculosis (TB) Screening form, every two years from the date of the first screening.
Caregiver # 1's TB screening was not completed every two years. The screening expired 2 months ago.
Plan of Correction: A current TB screening will be obtained.
