During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 09/02/2025 and completed on 09/02/2025. The on-site inspection began at 10:00 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 3 caregivers; the point total was 23. The inspector reviewed 4 children?s records and 6 caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/12/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Caregiver #2, who had been employed for more than 6 months, had a criminal background check dated more than 30 days after hire.
Caregiver #3, who had been employed for more than a month, had a criminal background check dated more than 30 days after hire.
Plan of Correction: I will make sure the background are redy before they start.

The provider must request a search of the central registry prior to the employee's first day of employment.

Caregiver #2, who had been employed more than 6 months, had a central registry search more than 2 months after the hire date.
Caregiver #3, who had been employed more than 1 month, had a central registry search more than 3 weeks after the hire date.
Plan of Correction: I will make sure everything is in order before start date.

Assistants' and substitute providers' records shall contain the following: documentation that two or more references as to character and reputation as well as competency were checked before employment.

Caregiver #2 has been working for over 4 months without documented references.
Caregiver #3 has been working for over 1 month without documented references.
Plan of Correction: I will make sure to ask fo reference.

The provider shall obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening.

Caregiver #3 has been employed for over a month and did not have documentation of a TB test or screening.
Plan of Correction: I will make sure they have the Tb test done before work.

The tuberculosis form shall have been completed within the last 30 days and be signed by a physician, physicians' designee, or an official of the local health department.

Caregiver #2 had a TB test dated over 6 months before the date of hire.
Plan of Correction: I will make sure to have the documentations ready.

REPEAT VIOLATION

When meals or snacks are provided by the family day home, the menu for the current one week period shall be dated.

The menu was not dated for the current week.
