An unannounced monitoring inspection was initiated on 10/28/2024 and completed on 10/28/2024, in response to a self-report received by the licensing office on 10/14/2024 relating to 22.-289. 031.B2. The inspector reviewed 1 children?s record and 3 staff records on site 10/28/2024. This inspection included document review, tour of the facility, interviews, and measurements. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
