Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in-person tour of the program.
An in-person renewal inspection was initiated on 2/28/2022 and the virtual component was concluded on 03/07/2022. There were 12 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and required staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, a household member did not obtain a Central Registry Search prior to the expiration date. Evidence: Expiration date for Central Registry Search for household member #1 was 1/23/2022.
Plan of Correction: The Provider obtained the required documentation and submitted it on 3/1/2022.
