Inspector Notes

A monitoring inspection was conducted on 10/17/2022 between the hours of approximately 08:45 am and 11:00 am. There were 11 children present, ranging in ages from 15 months to 4 years old, with 3 caregivers supervising.
Children were observed while working on their morning learning centers and circle time. Children were engaged in completing puzzles, tracing, coloring and working with manipulatives on sequence and linking. During circle time they reviewed the days of the week and months of the year, the alphabet, numbers, counting up to 100 while singing and exercising. These activities were done in English, Spanish and some ASL. Younger children also participated of the circle time and followed along on the activities. Assistants supported the provider by helping supervise the children, organizing materials, and helping children with the bathroom and handwashing.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 3 staff records and 1 household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview, it was determined that the provider did not ensure that caregivers that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Central Registry search request within 30 days of employment.
1) The record for Staff #3 (start date 06/23/2022) did not contain documentation of an out-of-state Child Abuse and Neglect search request. Staff #3 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.

Based on record review, the provider did not obtain for each adult household member a current Report of Tuberculosis (Tb) Screening form every two years.
