Inspector Notes

This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 04/13/2021 and concluded on 04/20/2021. The provider was contacted by telephone to initiate the inspection. There were 10 children, 2 staff providing direct care and supervision and 2 support staff present during the inspection. The inspector reviewed 2 children's records and 2 staff records during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard 22VAC40-185-240-D-5
Based on record review and discussion with the staff, the facility failed to ensure that there shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

Evidence:
1. The record for Staff #1 contained documentation of completing a daily health observation course on 09/09/2015. There was not a follow-up course for 2018 nor has a follow-up course been completed since 09/09/2015.
2. Staff #2 has not completed a daily health observation course.
Plan of Correction: The volunteer school nurse will be asked to teach a daily health observation course.
Standard 22VAC40-185-280-G
Based on observations and discussion with the staff, the facility failed to ensure that if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents and shall not resemble food or beverage containers.

Evidence: A plastic spray bottle in the classroom was labeled "sanitizing spray" and did not clearly indicate the contents of the spray bottle. When asked what the sanitizing solution was, the staff stated that the plastic spray bottle contained a purchased sanitizing agent. Staff also stated that the sanitizing agent came in a large container and that the original container was kept in a storage area.
Plan of Correction: The plastic spray bottle was labeled with the sanitizing agent product name during the inspection.

Based on observation and discussion with the staff, the facility failed to ensure that the first aid kit contained the minimum requirements as stated in the standards.

Evidence: The first aid kit was missing a triangular bandage.
Plan of Correction: Triangular bandages will be purchased.
