Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 10/04/2021 and concluded on 11/09/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 13 children, ages 3 years-5 years, 2 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to ensure that at least every two years from the first initial tuberculosis screening or testing, staff shall submit the results of a follow-up tuberculosis screening/test.

Evidence: The record for Staff 2 had a tuberculosis screening/test dated 09/18/2019; a 2021 follow-up screening/test has not been obtained.
Plan of Correction: Will call and schedule a test/screening today.
