Inspector Notes

An unannounced renewal inspection was conducted on 03/24/2022. There were 14 children, ages 3 years-5 years, 2 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. The children were observed during center play, participating in learning time and in free large motor play. A total of 5 children?s records, 3 staff records, 5 Board Officer/applicant records and 1 medication were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation and discussion with staff, the center failed to ensure there shall be a posted daily schedule.

Evidence: There was not a daily schedule posted during the 03/24/2022 inspection.
Plan of Correction: Will post one.
Standard 8VAC20-780-40-K
Based on discussion with staff, the center failed to develop written procedures for prevention of shaken baby syndrome or abusive head trauma, that include coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Evidence: The center?s Abusive Head Trauma procedures did not include information regarding coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Plan of Correction: Will develop the procedures.
Standard 8VAC20-780-60-A
Based on record review and discussion with staff, the center failed to ensure that the separate record for each child enrolled contains all of the elements as required by the standard.

Evidence:
1. The record for Child 3 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for the designated people are incomplete).
2. The record for Child 5 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (no addresses were provided for the designated people).
Plan of Correction: Will contact parents and get the information.
