Inspector Notes

An unannounced monitoring inspection was conducted on 02/28/2023. There were 12 children, ages 3 years-5 years, 2 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during free choice activities, story time and various learning activities. A total of 5 children?s records and 2 staff records were reviewed. The inspector discussed background check requirements with staff. The inspector arrived to the center at 8:45 am and departed at 10:30 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the center failed to ensure that all employees shall undergo background checks in accordance with the Code of Virginia prior to employment and every five years thereafter.

Evidence: The most recent criminal history check for Staff 2 was dated 02/21/2018; there was not a repeat criminal record check obtained by 02/21/2023.
Plan of Correction: Will set up an appointment to get new fingerprints and will check all other staff records.

Based on document review, the center failed to implement monthly practice evacuation drills.

Evidence: There was not a practice evacuation drill conducted in January 2023.
Plan of Correction: Will ensure that drill are done monthly.
