Inspector Notes

An unannounced renewal inspection was conducted on 03/20/2024. There were 11 children, ages 3 years-5 years, 2 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during music time, learning time, during large motor play in the gym, and during lunch. A total of 5 children?s records, 2 staff records and 3 applicant records were reviewed. The inspector discussed nutrition and annual training with the staff. The inspector arrived at the center at 9:15 am and departed at 11:10 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the provider failed to ensure that all caregivers shall undergo a background check in accordance with the Code of Virginia prior to employment and every five years thereafter.

Evidence: The most recent sworn statement in the record for Agent 3 was dated 03/15/2018; there was not a follow-up sworn statement completed by 03/15/2023.
Plan of Correction: A new sworn statement was completed during the inspection.
