During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated on 03/20/2026 and completed on 03/24/2026. The on-site inspection began at 9:10 am and ended at 10:50 am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 staff. The inspector reviewed 2 children?s records, 2 staff records, and 5 applicant records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4The most recent sworn statement for Applicant 1 is overdue by 7 months. The most recent central registry search for Applicant is overdue by 6 months. The sworn statement for Applicant 2 was not updated within 5 years of the original document being signed; it was signed 6 months passed the required timeframe. The criminal record check for Applicant 5 was not updated within 5 years of the original check being obtained; it was obtained 7 months passed the required timeframe.
Staff 2 did not complete the required 16 hours of annual training hours.
There was not a staff member with the group of children that has a current certification in CPR and first aid.
The injury report for Child 1 did not contain all of the components required by the standards.
