Inspector Notes

A renewal inspection was conducted on April 29, 2022. There were 68 children present directly supervised by 10 staff. The children were observed in individual play, center activities, music, and outdoor play.

Limited monitoring of child care requirements occurred during today?s visit. Licensing inspector monitored supervision of children, physical plant, staff/child ratios, adequate and approved space, programming, and nutrition.

The inspector reviewed on 5 child?s record and 8 staff records.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff record review, the center did not obtain documentation of criminal history records prior to the 1st day of employment for staff.
Evidence:
Staff #5 (date of hire 1/25/2022) had criminal history records dated 2/3/2022.
Plan of Correction: The first week of the staff #5 employment was a training period held online. The history of criminal records was obtained on 2/3/2022 prior to the first day of the in-person work.

Based on staff record review, the center did not have documentation of out of state child abuse and neglect results.
Evidence:
Staff #2 did not have documentation of out of state (MD) child abuse and neglect results during inspection.
Staff #5 did not have documentation of out of state (MD) child abuse and neglect results during inspection.

Based on staff record review, documentation of the TB screening was not submitted at the time of employment.
Evidence:
Staff #3 (date of hire 02/07/2022) had a TB test dated 02/22/2022 during the inspection.
Plan of Correction: Staff member #3 had online training and started working onsite after receiving the TB screening result on 2/25/2022.

Based on staff record review, the center did not have TB screenings at least every two years from the date of the initial screening or testing.
Evidence:
Staff #7 had a TB test dated 3/11/2020 (Expired 3/11/2022) on file during the inspection.
Plan of Correction: The test result date 9/28/2021 has been provided on 4/29/2022.

Based on child record review, the center did not have documentation of viewing proof of the child?s identity.
