Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 10/20/25 from 10:30 am to 12:30pm. The inspector reviewed compliance in the areas listed above. There were 54 children present and 10 staff. The inspector reviewed 4 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-80-A
The center did not maintain for each group of children, written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
1. On this day the software platform used by the center was out of service. This platform is used to record daily attendance for children which could not be recorded due to the outage.
2. The center also did not have a written record of attendance for each child in care.
Plan of Correction: Attendance will be documented on a paper roster daily in addition to the software.
