Inspector Notes
A monitoring inspection was conducted on June 7, 2022 and concluded on June 7, 2022. On June 7, 2022, the licensing inspector was onsite from approximately 2:45 pm until 4:15 pm. The director assisted with the inspection. There were 12 children present, ranging in ages from 20 months to three years, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence:
1. The national fingerprint check for Staff #1, hired on 03/09/2022, was dated 09/02/2021 exceeding 90 days prior to employment.
2. The director reported Staff #1 was delayed in starting employment and a new fingerprints
background check was not completed prior to hire.
Evidence:
1. The record for staff #1, hired 03/09/2022, had documentation of a sworn statement dated 08/31/2021,exceeding 90 days prior to hire.
2. The director stated, staff #1 was delayed in staring employment and an updated sworn statement was not completed at the time of employment.
Evidence:
1. The record for staff #1, hired 03/09/2022, did not contain the results of a central registry finding. The record did not contain documentation that the central registry had been requested and the staff has been continuously employed.
2. The director stated that a central registry for Staff #1, had not been requested.
Evidence:
1. The record of staff #1, hired 03/09/2022, did not have documentation of two or more references as to the character and reputation as well as competency that were checked before employment.
2.The director stated two references for staff #1 had not been obtained.
