Inspector Notes

An unannounced monitoring inspection was conducted on 1/30/2024 from 9.28 pm to 10:40 am. There were two children in care, age three years, in one group under the care of one staff. The Licensing Inspector reviewed three children?s records, two staff records, and other program records. The children were observed in active structured play, singing, and having snacks.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center failed to obtain the results of a criminal record search before a new staff member began employment and failed to obtain the results of Central Registry searches prior to the 30th day of employment.

EVIDENCE:
Staff 3 has a documented employment date of 8/2023 (exact date not recorded). The results of the criminal record check is dated 9/14/2023.
Plan of Correction: From now on, the results of criminal record checks will be obtained before a new staff person begins employment.
Standard 22.1-289.058
Based on observation and interview, the center failed to have a carbon monoxide monitor in the building, which is required for buildings built after 2015.

EVIDENCE:
The LI did not see a carbon monoxide monitor, and staff 1 confirmed that there was none, and that the building was not built after 2015.
Plan of Correction: A carbon monoxide detector was ordered the day of the inspection.

Based on record review, the center failed to obtain complete background checks for staff members.

2. Staff 2 has a documented employment date of 7/31/2013 and the record does not contain the results of a Central Registry child abuse and neglect search.
Plan of Correction: From now on, the sworn disclosure statement will be completed before a new staff person begins employment. Central Registry checks will be requested in a timely manner.
The central registry search request process began the day of the inspection.
