Inspector Notes

An unannounced on-site monitoring inspection was conducted on 11/13/2023. There were 123 children, ages 3 months-5 years, and 19 staff members present during the inspection. The inspector observed the following: free choice activities , lunch and nap. A total of 6 children?s records and 6 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 10:30 AM and departed at 12:45 PM.

Based on observation, the center failed to ensure that all medications were locked.

Evidence:

On the diaper changing table an opened tube of prescription diaper cream was unlocked.
Plan of Correction: The medication was locked during the inspection.

Based on observation of the infant room, the center failed to ensure that children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

Evidence:

Observed child # 8 (11 months old) having a whole meatball for lunch. A meatball is considered a potential choke hazard for the size and shape of the piece of food.
