Inspector Notes

An unannounced on-site renewal inspection was conducted on 03/07/2024. There were 123 children, ages 2 months-5 years, and 20 staff members present during the inspection. The inspector observed the following: morning snack, free choice activities in the gym, and lunch. A total of 6 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: sending links from the DOE website. The inspector arrived at 10:00 AM and departed at 12:30 PM.

Standard 8VAC20-780-130-A
Based on review of children?s records, the center failed to ensure that all children had the immunization record prior to beginning attendance.

Evidence:

The record for Child # 4 contained documentation that the first day of attendance was 01/16/2024. The date of the immunization record was 02/16/2024.
Plan of Correction: The director in the future will make sure that all children starting will have the immunization record.

Repeat Violation

Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.

Evidence:

1. In the entrance way under the check in counter there was a bucket with two chemicals containers.
Plan of Correction: The chemicals were removed and locked during the inspection.

Based on observation, the center failed to ensure that all children under the age of 10 was in actual sight and sound supervision of the staff.

Evidence:

LI observed a speech therapist from a local school system walking child # 1 up stairs to provide services. The child was not logged out of the sign in and out sheet. Child # 1 was in the care of the center at the time and was not within sight and sound supervision.
Plan of Correction: The director will make sure that all children that needs outside services have the agreement completed. This agreement is between the outside agency, the parent and the center. The outside agency staff that is providing services will ensure that they sign the child in and out each time.

Based on interviews with staff, the center failed to ensure that all tables were sanitized before and after each meal or snack.

Evidence:

In the infant 2 room, the teacher stated that she uses Pinesol after the children eat. According to Pinesol?s directions it is to be used as a cleaning agent.
Plan of Correction: New cleaning and sanitizing chemicals were ordered, and director will make sure that they are being used correctly.
