Inspector Notes
An unannounced on-site renewal inspection was conducted on 03/07/2024. There were 123 children, ages 2 months-5 years, and 20 staff members present during the inspection. The inspector observed the following: morning snack, free choice activities in the gym, and lunch. A total of 6 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: sending links from the DOE website. The inspector arrived at 10:00 AM and departed at 12:30 PM.

4Evidence:
The record for Child # 4 contained documentation that the first day of attendance was 01/16/2024. The date of the immunization record was 02/16/2024.
Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.
Evidence:
1. In the entrance way under the check in counter there was a bucket with two chemicals containers.
Evidence:
LI observed a speech therapist from a local school system walking child # 1 up stairs to provide services. The child was not logged out of the sign in and out sheet. Child # 1 was in the care of the center at the time and was not within sight and sound supervision.
Evidence:
In the infant 2 room, the teacher stated that she uses Pinesol after the children eat. According to Pinesol?s directions it is to be used as a cleaning agent.
