Inspector Notes

An investigation was initiated on December 15, 2022 and a follow up visit was conducted on January 6, 2023 in response to a complaint that was received by the Fairfax office on December 14, 2022 regarding the center's physical plant. A self-reported incident was also received by our office on December 15, 2022, regarding special care provisions and emergencies. Based on staff interviews, records reviewed and observations made, the preponderance of evidence gathered during the investigation does not support the allegations. The complaint and self-reported incident are determined to be not valid. No violations were issued.
