Inspector Notes

An unannounced renewal inspection was conducted today from 11:10am-2:15pm. There were 144 children (infants-school-age) directly supervised by 37 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, eating lunch and napping. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, children's records did not include all required information.

Evidence:
1.Child #1's record did not include the name, address and phone number of two required emergency contacts to be contacted in an emergency.
2.Child #4's record did not include the address of two designated persons to be contacted in an emergency.
Plan of Correction: The parents of Child #1 were contacted 27 June 2023 and were asked to provide the name, address and phone number of 2 emergency contacts to be contacted in the event of an emergency and they were not available/could not be reached.
They responded and as requested will provide the required information before the close of business Friday, 30 June 2023.
The parents of Child #4 were contacted 27 June 2023 and were asked to provide the address of 2 designated persons to be contacted in the event of an emergency.
They responded and as requested will provide the required information before the close of business Wednesday, 5 July 2023.
