An unannounced investigation was conducted on 7/27/2023, and a follow interview was completed on 8/4/2023, in response to a complaint that was received by the licensing office on 7/20/2023 regarding staffing and supervision. Based on staff interviews, records reviewed and observations made, the preponderance of evidence gathered during this investigation does not support the allegations. The complaint is determined to be not valid.
