Inspector Notes

An unannounced investigation was conducted on 08/24/2023, and a follow up was conducted on 09/05/2023, in response to a complaint that was received by the licensing office regarding diapering requirements. Based on staff interviews, and review of documentation, the preponderance of evidence gathered during this investigation supports the allegation. The complaint is determined to be valid. Information gathered during the investigation determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. Based on interviews, center staff did not clean and change a child's diaper immediately when it became wet.

Evidence:
1. On 8/10/2023, Child #1 (2 years old) was not cleaned and changed immediately after their diaper became wet.
2. Staff #2 stated that Staff #1 checked diapers in the morning, but Child #1's diaper was dry. The diaper report sent to the parents through the center's app documented that Child #1 was checked at 9:44am and at 12:44pm, both times it was documented that the child had a dry diaper.
3. During the afternoon, Staff #1 stated that they didn't change/check diapers after naptime because they were taking the children to the playground. Staff #1 stated that when they returned from the playground, they began to check/change diapers, but they did not get to change Child #1's diaper because a parent arrived to pick up their child and they were sidetracked.
4. Once Staff #1 left the center at approximately 4:45pm, the child's diaper was not checked again which resulted in a completely saturated diaper. Child #1 did not have a diaper change from the time the child was dropped off at approximately 8:52am until approximately 5:38pm when the child was picked up from the center.
Plan of Correction: The parent of Child #1 made the FBCC CDC aware CHild #1 had not been changed on 10 August the evening of 10 August.
Staff #1 was interviewed about the mater the morning of 11 August.
Staff #1 recounted the events of 10 August as outlines in the evidence section of the violation notice including that Child #1's diaper although it had been checked at 9:44am and 12:44pm and found to be dry (and not changed) was not checked/changed after resulting in a soaked diaper.
The following plan to avoid future occurrence of the same or similar incidents was developed and put in place 11 August.
Caregivers - teachers will continue to change children's diapers immediately when a child is wet or dirty.
Caregivers - teachers will develop a system to insure children's diapers are checked and changed at scheduled times to eliminate the possibility any child's diaper is not checked AND changed even if/when the diaper is dry and/or clean.
To insure to the best of each caregiver's - teacher's ability each child's health, safety, welfare and well-being this plan will be monitored/reviewed and modified as needed so parents are assured their children's diapers are being changed.
