An unannounced investigation was conducted on 11/30/2023 in response to a complaint that was received by the licensing office on 11/14/2023 regarding staffing and supervision. Based on staff interviews the preponderance of evidence gathered during this investigation does not support the allegation. The information gathered during the investigation determined no violations with applicable standards or law. No violations were issued.
