Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on 03/10/2022 and concluded on 03/10/2022 The director/provider or in-charge person was contacted by telephone to initiate the inspection. There were 20 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records (includes 1 medication record) and 4 staff records (includes (includes background checks for business entity & staff identified at MAT certified), along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review & discussion with the Center Director, the Licensee failed to ensure MAT certified staff are retrained at three year intervals.

EVIDENCE:
Medication administration training for Staff #4 expired December 2021. Licensing Inspector observed multiple prescribed medications during today's inspection.
