Inspector Notes
An unannounced on-site monitoring inspection was conducted on 04/25/2023. There were 26 children, ages 3-6 years, and 4 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, handwashing and snack. A total of 5 children?s records and 4 staff records were reviewed. The inspector discussed the following with staff: orientation, cleaning chemicals, staff record keeping and attending a Phase 2 training in the Office of Child Care Health and Safety. Due to number of violations related to background checks, an Intensive Plan of Correction (IPOC) will be required. The inspector arrived at 9:40 AM and departed at 12 PM.

1.Staff # 1 had a documented employment date of 08/07/2022. The fingerprint letter of eligibility was dated 09/10/2022.
2.Staff # 4 had a documented employment date of 08/01/2022. The fingerprint letter of eligibility was dated 01/25/2023.
Evidence:
Staff # 2 had a documented employment date of 08/05/2022. Staff # 2 indicated on the sworn statement that they have lived outside of the state of Virginia in the past five years. An out of state child abuse and neglect search request was not in the record.

1. Staff # 1 had a documented employment date of 8/7/2022. A completed search of the central registry was not in the record. Staff #1 was observed working with children during the inspection. Staff # 3 was unable to provide documentation when requested by the inspector.
2. Staff # 4 had a documented employment date of 11/17/2022. A completed search of the central registry was not in the record. Staff #4 was observed working with children during the inspection. Staff # 3 was unable to provide documentation when requested by the inspector.

2.Staff # 4 had a documented employment date of 08/01/2022 and the two references were dated 11/04/2022 and 11/15/2022. The two required references were to be obtained prior to hire.

2.Staff # 4 had a documented employment date of 8/1/2022 and the date of the TB screening/test was on 01/21/2023.
