Inspector Notes

An unannounced on-site renewal inspection was conducted on 03/01/2024. There were 11 children, ages 2 1/2 months-5 years, and 2 staff members present during the inspection. The inspector observed the following: morning snack, free choice activities, and large motor play outside. A total of 6 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: sending links from the DOE website. The inspector arrived at 9 :40 AM and departed at 11:30 AM.

Standard 8VAC20-780-240-A
Based on review of staff?s records, the center failed to ensure that all staff completed the Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Evidence:

Staff # 1 and # 2 did not have documentation of the Virginia Department of Education-sponsored orientation course (Preservice 10-hour training).
Staff # 1 first day of employment was 08/23/2023 and staff # 2 first day of employment was 08/01/2022.

Standard 8VAC20-780-280-B
Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.

Evidence:

1.A spray can of Lysol was sitting on the shelf above the sink outside the restrooms. The chemical was unlocked.
2.The cabinet in the kitchen above the sink was not locked and contained 7 containers of chemicals.

Based on review of children?s records, the center failed to ensure that records and reports on children shall be maintained and made accessible for two years after termination of services or separation from employment.

Evidence:

Record for child # 1 was missing the original immunization and physical form. Staff # 4 stated that the lead teacher obtained updated immunization and physical form and shredded the original immunization and physical.

Based on observation, review of two staff records, and interview, the center failed to
ensure that all classrooms had a staff person with First Aid and CPR certification.

Evidence:

Staff # 1 was working alone in the in the main classroom area and the staff?s record did not
have documentation of First Aid and CPR Certification. Staff # 1 was cited on 10/26/2023.

Based on observation, the center failed to ensure that all food brought from home was dated and labeled with the owner?s name.

Evidence:

The LI observed the date of 2/6 documented on the lunches on the day of the inspection.
Plan of Correction: The director will make sure that staff change the date every day.

Based on review of children?s records, the center failed to ensure that a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

1.Child # 6 has a diagnosed food allergy and the plan in the record was not completed by the child?s physician.
