Inspector Notes
An unannounced on-site renewal inspection was conducted on 03/01/2024. There were 11 children, ages 2 1/2 months-5 years, and 2 staff members present during the inspection. The inspector observed the following: morning snack, free choice activities, and large motor play outside. A total of 6 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: sending links from the DOE website. The inspector arrived at 9 :40 AM and departed at 11:30 AM.

Violations
7Evidence:
Staff # 1 and # 2 did not have documentation of the Virginia Department of Education-sponsored orientation course (Preservice 10-hour training).
Staff # 1 first day of employment was 08/23/2023 and staff # 2 first day of employment was 08/01/2022.

Evidence:
1.A spray can of Lysol was sitting on the shelf above the sink outside the restrooms. The chemical was unlocked.
2.The cabinet in the kitchen above the sink was not locked and contained 7 containers of chemicals.
Evidence:
Record for child # 1 was missing the original immunization and physical form. Staff # 4 stated that the lead teacher obtained updated immunization and physical form and shredded the original immunization and physical.
Evidence:
There was a bottle of Zyrtec for child # 6 that was unlocked in the cabinet above the kitchen sink.
Based on observation, review of two staff records, and interview, the center failed to
ensure that all classrooms had a staff person with First Aid and CPR certification.
Evidence:
Staff # 1 was working alone in the in the main classroom area and the staff?s record did not
have documentation of First Aid and CPR Certification. Staff # 1 was cited on 10/26/2023.
Evidence:
The LI observed the date of 2/6 documented on the lunches on the day of the inspection.
Evidence:
1.Child # 6 has a diagnosed food allergy and the plan in the record was not completed by the child?s physician.
