Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/5/2025 and completed on 5/5/2025, as a part of the licensure period. The on-site inspection began at 11:04AM and ended at 12:51PM. The inspector reviewed compliance in the areas listed above. There were 77 children present and 13 staff. The inspector reviewed 5 children?s records on site on 5/5/2025 and 1 staff records on-site on 5/5/2025. Medication was reviewed on site 5/5/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for child #3 was not signed.
Plan of Correction: Provider obtained singed immunization record.
Standard 8VAC20-780-510-E
The centers procedures for administering medication shall include long term prescription drug use may be allowed with written authorization from the parent. The long term medication authorization for Medication #4 did not have written authorization from the parent.
Plan of Correction: Provider obtained written authorization from parent for long term medication.
