Inspector Notes
A monitoring inspection was conducted on 06/04/2024 from approximately 12:45 p.m. to 3:50 p.m. There were 49 children present and 8 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five child (5) records and five (5) staff records were reviewed.
The information gathered during the inspection determined violations with applicable standards or law. Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.
Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Evidence:
The record for S2, employed 11/30/2022 contained a certificate of completion of the
Virginia Preservice Training dated 05/30/2023.
The record for S4, employed 08/30/2023 contained a certificate of completion of the
Virginia Preservice Training dated 12/03/2023.
Based on observation, the provider did not ensure that all areas of the center be maintained in a clean, safe, and operable condition.
Evidence:
A splintered board was observed on the top of the picnic table on the large playground that can snag children?s skin.
Exposed insulation was observed on the HVAC duct within reach of children in classroom #3.
Evidence:
The injury prevention procedures provided for review documented the last review dated 02/08/2023.
Evidence:
Licensing inspector observed classroom #3 during the rest period and multiple children rest mats were less than 12 inches apart.
Evidence:
Licensing inspector observed the school age classroom children have a snack after watching movie at the same table without the table being sanitized before the snack and afterwards.
Based on review of children records, the provider did not ensure that one of five children?s records contained the required information.
Evidence:
The record for child, C2 enrolled 05/06/2024 did not document two designated people
to contact in case of an emergency if the parent cannot be reached.
