Inspector Notes

An unannounced renewal inspection was completed on 12/06/2024. The on-site
inspection began at approximately 11:50 a.m. and ended at 4:00 p.m. The inspector reviewed compliance in the areas listed above. There were 32 children present and 7 staff supervising. The inspector reviewed 5 child records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/20/2024. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. S5 who had been employed for one day, did not have a completed national criminal background check until the following day. The file for S5 documented the staff receiving orientation and training required 8VAC20-780-240-C the day prior to the national criminal background check being completed.

Standard 8VAC20-780-140-B
If a child has had a physical examination prior to attendance, it shall be within the time period prescribed. The physical examination for C3 who is under the age of 18 months was not within the required three month prior to attendance at the center.

Immunization records and physical exams are required to be signed and dated by a physician, his designee, or an official of the local health department. The physical and immunization record for C2 was not signed. The immunizations dated 10/15/2024 for C3 were not signed. The immunizations dated 05/03/2024 for C5 were not signed.

Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside,
are maintained in a clean, safe and operable condition. The laminate on the sink used by children in the two-year-old room is peeling and has exposed particle board. The floor laminate on the ramp to the children?s bathroom in the two-year-old room has separated from the wood ramp creating a trip hazard.

In areas used by preschool age or younger children, electrical outlets shall have protective
covers. A power strip in the one-year-old room providing power to the portable sink did not have protective covers and was within reach of children.

During the designated rest period the rest period ratio for children 16 months through 24 months is one staff per 10 children. Licensing inspector observed 2 staff supervising 12 children in the one-year-old classroom room during the rest period; however, the youngest child in the room was not yet 16 months causing the class to be out of ratio.

Standard 8VAC20-780-500-B
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff in the 2-year-old room cleaned the diaper changing surface after diapering a child and sprayed it with a bleach water solution, but did not allow it to dry for a period of at least two minutes.

The center shall maintain a written record of children?s serious or minor injuries that contain all the required information. Injury reports dated 07/08/2024 and 08/16/2024 did not document the date and time the child?s parent was notified of the injury. Injury reports dated 10/11/2024 and 12/23/2024 did not document the time the parents were notified of the injury.

Repeat Violation
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for C4, enrolled for over 3 months, was missing the following required component: documentation of viewing proof on the child?s identity and age.
