A subsidy health and safety inspection (SHSI) was conducted 12/06/2024 in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/20/2024. A POC submitted after this date will not appear on the public website.

For each group of children, the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. On 12/06/2024 the licensing inspector observed 32 children in the center. The daily attendance record documented 27 children present. The daily attendance log for 11/27/2024 did not document the departure time for 7 children. The log for 12/02/2024 did not document the departure time for 12 children. The log for 12/03/2024 did not document the departure time for 7 children. The log for 12/05/2024 did not document the departure time for 7 children.
