An unannounced monitoring inspection was conducted on August 15, 2023. This on-site visit was conducted from 10:30 a.m. through 11:40 p.m. Upon arrival, there were 20 children, ages 3 years through 5, in the care of 4 staff members. Five children and ten staff records were reviewed.

Based on record review the facility failed to have documentation of an updated sworn statement for one staff within the last five years.
Evidence:
1. Ten staff records were reviewed.
2. Staff # 6 did not have documentation of completing a sworn statement within the last five years. Last signed statement was documented on 08/13/2023.
3. The director confirmed Staff #6 had not completed an updated sworn statement.
Based on record review the facility failed to have documentation of obtaining a updated fingerprint checks for one staff within the last five years.
