An unannounced monitoring inspection was conducted on Auguste 20, 2024. This on-site visit was conducted from 11:30 a.m. through 12:15 p.m. Seventeen children, ages 3-6, were in the care of four staff. Five children and three staff records were reviewed.

REPEAT VIOLATION
Based on record review, the religious exempt facility failed to obtain an repeat central registry check for one staff member.
Evidence:
1. Four staff records were reviewed.
2. Staff #1 did not have documentation of a repeat central registry check within the past five years. Central registry was dated 12.12.2018.
3. Administration confirmed a repeat central registry had not been obtained within the last 5 years.

Plan of Correction: The repeat central registry check was sent on 06.12.2024. Results have not been received. An email to the office of background was sent for a response. The central registry will be resubmitted by 8.22.2024 if not response has been received. Documentation will be sent to the licensing inspector.
