During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on March 30, 2026 and completed on March 31, 2026. The on-site inspection began at 11:15am and ended at 12:05pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 9, 2026. A POC submitted after this date will not appear on the public website.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

1. There were damaged floor tiles that provided a possible tripping hazard in the hallway and in the girl?s bathroom.
2. In the girls bathroom, behind the toilets, there was missing tile.
3. A toilet seat in the girl?s bathroom had multiple areas where the with chipped paint.
Plan of Correction: The tile in the girls bathroom and hallway has been replaced. The tile behind the toilets in the girls bathroom has been replaced. The toilet seat in the girls bathroom has been replaced.
